RESOLVED, That the proper City officials are hereby
authorized and directed to execute an agreement with the
Hallie Q. Brown Community Center for the lease of space
at the Martin Luther King, Jr. Neighborhood Service Center
for the purposes of the City conducting a general health
clinic therein, subject to the terms and conditions of
said agreement, a copy of which is to be kept on file and
of record in the Department of Finance and Management

� This Lease Agreement entered into this day of ,

to as the "Administrator" and City of $aint �Poul, Division of Public Health ,
hereinaf ter referred to as the "Agency". • �
WITNESSETH: -
That said Ac�inistrator, in consideration of the covenants cantained
in this agreement, and for other good an3 valuable consideration, the receipt
and sufficiency whereof are hereby acl�rw��aledged, hereby agrees with the Agency
as follows: •

1. The Administrator hereby leases to the Agency space in the .
. Martin Luther King, Jr. I�Teighborhood Service Center, hereinafter referred ta
as "Center", located at l�arshall and Kent Streets in the City of Saint Paul,
State of Mi.nnzsota, designated Winq C , corrtaini.ng 3,431 �
square feet, hereinafter referred to as the "Premises", according to a plan
attached hereto as �chibit A, said space being the shaded portion thereof,
for the term of one �ear from the 1 st day of January , 19 82 , for the

2. Ac�ministrator sha1J. provide maintenance, utilities, except tele-
phone, and such office furniture as Ac�ni.ni.strator may have avaa.Zable for use
by Agency, and furt.her shall provide reception answerino service.during ttx�se
l�urs that ths Center is open, Ac�mi.nistrator also grants to Agency the use of
the conferencs room, rest rooms and other pubZie axeas and facilities avai.Iabl� �
to a�zd used by other tenants in the Center.

��= . 4. Agency sna11 pay all costs relating to telephone servi.ce, all
salaries and wages for its em�ployees, and shall not incur any further or
additional e.cpense with xespzct to the Premises.unless specifically author-
� ized by Admini.strator in wx•; _�.ng.
5. Agency agrees :.t its staff wi.11 cooperate.with tra� Admi.n-
istrator and other tenants �� .: personnel of the Center to the end that an
integrated program wi1Z ecn�:�ate from the Center, and the comrminity will re-
' ceive the best possible service and assistance from the Center, inc�uding
cooperation by the parties and other tenants of the Center in the d�velopssnt
o£ a central registration system of inclividuals using and being serviced.by
the Center. . � �
� 6. Agency agrees to provide representatives in such number as '
may be hereafter mutually agreed upon to attend al�. general staff ineetings
of the Center, and to participate in case conferences caZled by the Center
coordinator. -
7. T'ne Agency agrees to furnish the Administrator with such
repoxts azd records as Administrator �.y deen desirable for determini.no
the use bPino made by Agency of the Center, the effectiveness of the pr�grams
emanating from Center, and in planr�.ing neta programs, except that Agency shall
not b� rernaired to provide any depth xecords of a highly confidential nat-�:re. ..
8. Agency a�rees not to post in the Center any external si�s,
pictures, ac��ertise�aents, banners or decoratior.s of any kir,d oz typz �rithout
the prior written consent of the Administrator.
' 9. Agency aorees to keep the Preraises in an orderly, clean and �
sax�itary condition; will neither do or p2xmi.t to be done therein anythi.ng
which is .in violation of the te r�ns of any insurance policies on the Center;
� will neither do nor pexmit to be done therein anything in violation of the
laivs, regulations or ordinances of any governmental body applicable thereto,
. and will not make any alterations, additions ar chanoes therein tiY•ithout the
prior written consent of the Ac�ministrator.
10. In the event of any ciispute axising under the terms of this
agreement, such dispute sha11 be stated in writing to the other party, and
shall be f�rst resolved by the Center coordin�tor and staff m,ember of th�
Iigency so assigned b}• the Apenry� and if thexafter unresolved, �by the
coorclinator and the stipen�isor of the said st�ff ��naer, �sid if tnerea£ter

thirty (30) days therea{'ter to agree in �,rrzting to restore the s�e �rithin

PURPOSE AND RATIONALE FOR THtS ACTION= �ND MAIVAGE�ENT: SERV#CFS
The Model Citi�s Health Center has been located at the' Mc�rtin Luther King, ;Jr., �
Neighborhood Service C�nter foc nearly 1�en years. This is a nzw 1�e t�nt will
enable Model Cities Health Center to continue operations at the Martin Luther
King building.
f I NANCIAL IMPACT:
The 1982 lease expense is �17,155. Payment wi11 be mad� from the federal llrban
Health lnitiative grant. The lease is for a one-yecar p�riod.
