WHEREAS, the City Council of the City of Saint Paul, acting as the Board of
Health, has determined that the operation of the Model Cities Health Project is essential
to the health service mission of the City of Saint Paul, and that effective governance
and management of the Model Cities Health Project is best provided by an independent
board composed of area residents and providers; and
WHEREAS, the City Counci I wishes to recognize and encourage the expressed
intent of the board of directors of Model Cities Health Center, Inc., to govern and
manage the Model Cities Health Project and to adopt bylaws to describe a governing
board whereby said board will ultimately assume the full authority and responsibility to
conduct the activities of the Mode) Cities Health Project, a role presently performed
by the City of Saint Paul; and
WHEREAS, the City Council is determined to expedite this transfer of authority
in order that Model Cities Health Center, Inc., may assume as soon as possible the
position of sole grantee of the existing grant to the City of Saint Paul from the United
States Department of Hea Ith and Human Services;

NOW, THEREFORE, BE IT RESOLVED that the City Council advise Region V,
United States Department of Health and Human Services, of its intention to transfer
all interests in the Model Cities Health Project, Urban Health Initiative Program,
grant number 05 H 000904 04, to the successor organization, Model Cities Health
Center, Inc., at the earliest possible date.
