WHEREAS, the City of Saint Paul presently provides group hospital and medical
covera.ge programs for its �mployees and employees of Independent School District No. 625; and
WHEREAS, under this program, the City has a contract with Coordinated Health Care,
Incorporated to provide Hospital and Med.ical Surgical coverage, a.nd
WHEREAS, pursuant to the terms of this contract Coordinated Health Caxe, Incorporated
has submi.tted renewal rates for the covera.ge for the year, October 1, 1983 to September 30, 198�+
as follows:

to be identical to the coverages now provided under the existing contract be renewed to Coordi-

portion of the premiums as is agreed upon by the process of collective bargaining and the
employee shall pay the balance of premiums due through payroll deductions, and be it
FURTHER RESOLVID, that the proper city personnel be di.rected to pay these premiums
as they fall due, and be it
FINALLY RESOLVID, that the City Clerk shall be instructed to send a copy of this
resolution to Coordinated Health Care, Incorporated and the Ochs Agency, Incorporated.
