WHEREAS, the City of Saint Paul now provides Optional Life Insurance for its
employees and employees of the Independent School District �625 at group rates through
Minnesota Mutual Life Insurance Company of Saint Paul, now therefore be it
RESOLVID, that the present life insurance contracts #1520-G ana. #365379-G between
the Minnesota Mutual Life Insurance Company and the City of Saint Paul and Independent School
District �625 be continued, in their present entire forms, for the year, October 1, 1983 to
September 30, 1984, and be it
FURTHER RESOLVID, that the premi.ums for said contracts for said period are to be
as follows:

FURTI�t RESOLVED, that in order to stabilize experience and provide for a more
consistent pattern of probable dividend returns, the Minnesota Mutual will continue to allocate
paxt of the gross dividend available, not exceeding 10% of premium received. in any one year,
to a stabilization reserve on such optional insurance issued in excess of �15,000 until such
reserve reaches a level equal to 50°fo of the current annual premium. Minnesota Mutual credits
interest to stabilization reserves, at a rate of 9°�o annually for the yeax 1.983, and be it
FINALLY RESOLVID, that the City Clerk be instructed to send a copy of this resolution
to the Minnesota Mutua.l Life Insurance Company and Ochs Agency, Incorporated.

HEALTH AND WELFARE
1. Blue Cross rates include Aware and Non-Aware Contracts.
The contract has the following changes: �
a) Aware Program (exclusive use o£ member hospitals) is installed for most bargaining
units.
b; Outpatient surgery for some procedures is required.
c) Foundation for Health Care Evaluation will continue to monitor hospital admissions and
stays.
2. HMOM changed contract.
a) Co-payment for drugs increased $2.00 to $3.00. ,
b) Ambulance benefit increased $50 to $300.
c) Hospital emergency room co-payment increased from $15.00 to $25.00, (waived if
ad�itted to hospital.)
3. Physicians Health Plan (PHP) and Med Center Health Plan have been added as two additional
choices providing grepaid health care for bargaining units that include them in their
contracts.
4. St. Paul Life Insurance Company reduced the rate for Short Term Disability from $2.15 to
$2.04 per $100 benefit. The benefit option is increased to $900/month ($100 open without
evidence unless previously declined) . Long Term Disability contract is available as an
optional coverage with evidence of good health.
5> Spouse life insurance may be applied for in the amounts up to 50% of the total optional
life insurance in effect on the employee. (Evidence of insurability is required.)
Open enrollment (when you can change coverage without health questions) will start September 1,
1983, for October lst effective date. P.H.P, will require evidence.
For School District employees the open enrollment will start October 1, 1983, for effective
date of November lst. For change effective October lst, call the Ochs Agency, 298-3789.

THE OCHS AGENCY
P.epresentatives of the Ochs Agency, which service City and School District group insurance
p�ans, are available to provide the following services:
- Explain available coverages to new employees and take applications
•� Review each employee's present plan, explain alternative 'coverages available, take appli-
cations to add, change or delete coverage
- Ciarify claim procedures and provide help in resolving complicated claims
- Take address and name changes
- Assisr with beneficiary changes
- Secur.e r.ew identification cards and contracts
