Date form was completed Print the Name of the Person who Comp ted this orm:1'1 ."n c_IQ C L! Signature of Person Making the Claim: ; Revised February 2011 St Paul Regional Water Services knocked on our door Saturday evening, February 15th 2014 at 4:00 pm. They said our next door neighbors(the house to our south) had frozen pipes and that they would need to run electricity through their pipes to fix the ptoblem. Because of this, they said woutd need to come into our house to work on our water main. They disconnected the water main, which took about 10 minutes and then left to work outside.About an hour later when they were finished,a worker from the St Paul Regional Water Services came back into our house, connected the water main and turned the water back on.We were upstairs while this was occurring downstairs. The serviceman came back upstairs, telling us that everything was reconnected and that the water was back on.About 10 minutes after the Serviceman left our house my husband heard a sound coming from the other side of the house. After trying to locate the sound for a couple minutes, my husband eventually followed it to our basement bathroom and found that the water-line to the back of the toilet had been blown off and was flooding our bathroom, computer room and family room. He immediately yelled for me and told me to catch the serviceman before he left. I ran outside and flagged down Tony Palumbo. He was the first and only serviceman to come into our house to see the damage. By the time he was downstairs, my husband had turned the water off at the main.Tony Palumbo took some early pictures of the damage. He agreed with us that the pressure being blown through the pipes, when the city turned the water back on,was too much for the downstairs toilet and caused the fitting to pull loose, thus flooding the basement.Tony Palumbo said that they could get a wet/dry vac and try to clean up but my husband said not to bother since we have a wet/dry vac.We did vacuum the carpet as best we could but the padding under the carpet was too wet to be saved. Damage: 1. Bathroom: a. Bathroom tile is damaged b. Bottom of bathroom vanity is water damaged. c. Trim in bathroom is damaged. 2. Computer Room: a. The carpet/padding is water damaged. b. Trim piece was water damaged. 3. Family Room a. Carpet/padding is water damaged b. About 20 ft of trim is damaged c. About 20 ft of drywall is damaged Why we are submitting a claim/responsibility of damages We are expecting the city of St. Paul to reimburse us the complete amount of one of our 2 estimates to restore our bathroom, family room, and computer room to their previous living conditions. The city is responsible for all damages that happened to these areas of my home. Our two estimates are attached. In addition to the damages to my home, I am also expecting the city of St. Paul to pay for the following: a. The disposal of my carpet. I had to pay my garbage company$60 b. The city should reimburse me for a new bathroom vanity. The water damaged the whole bottom of the vanity and it is not fixable.A vanity that is comparable to the one we have is $200. c. I also feel that we should be compensated for the inconvenience of this whole matter and the time taken to deal with this mess to this point. Half of our basement is,for the most part, unlivable. We have had to take time to clean up water, rip up carpet and move everything out of the basement. We also had to take time to get two bids from two separate parties for the work involved.Together,my husband and I have so far spent about 15 hours of our time due to this mess. Before the project is over, I would estimate we would put in another 30.At the new minimum wage proposal of$9.50 that would come to$427.50. We would not have had to deal with any of this, had this never happened. BROSTROM CONTRACTING IIVC.
