Please submit either a resolution from your Iocal unit of govern-
ment or a written denial from ti�em. If a resolution is received '
please make the necessary changes, designated authorized official 's
name and signature on paoes 1 and 4 of the application. Then re-
turn three (3) copies of the corrected pages so that we may con-
tinue processing .your application.
