RESOLVFD, That upon execution and delivery of a release in full
to the City, in a form to be approved by the City Attorney, the
proper City officers are hereby authorized and directed to pay
out of the Tort Liability Fund 09070-511-000 , to:
1 . Petra Cusik the sum of $1 ,619 .84 in full settlement of
her claim of damages sustained on or about June 26 , 1987 , as
more particularly set out in the Notice of Claim filed in
the City Clerk ' s office on July 2 , 1987.

M E M O R A N D U M
CLAIM AGAINST CITY OF SAINT PAUL
BY PETRA CUSIK
Claimant , Petra Cusik , seeks reimbursement for damages to her
automobile which she incurred when it was struck by a Saint Paul
Public Works vehicle driven by Gerald Klemenhagen. The accident
occurred on June 26 , 1987 on Interstate 94 near Western Avenue
in Saint Paul . Mrs . Cusik had to stop her vehicle due to traffic
conditions and Mr . Klemenhagen was following too close�y and was
unable to stop in time to avoid hitting Mrs . Cusik ' s vehicle .
This office approves the settlement of Mrs . Cusik' s claim in
the amount of $1 ,619 .84 .
