• 1988 AGREEMENT FOR MATE NAL AND CHILD HEALTH SERVICES
BETWEE RAMSEY CQUNTY
AND THE ST. PAUL DIVISION OF PIIBLIC HEALTH
THIS RGREEMENT which shal be interpreted pursuant to the laWS ' of
Minnesota, entered into and exe uted this ________ day of ________, 1988,
between the County of Ramsey ac ing through its Public Health Department -
thereinafter Gounty? and the St. aul Division of Fublic Health (hereinafter
City) , witnesseth that:
WHEREAS, pursuant to the M ternal and Child Health Att, Minn. Stat.
145.88 et seq. and Minn Rule 461 .210(1 et seq. , the County has identified a
certain population within the C unty who are in need af family planning
services and wishes to purchase t ese �ervices from the provider, and
WHEREAS, The City represen s that it is qualified to furnish these
services; and
WHEREAS, Federal funds . ar available for the purthase of family
planning services under 42 U.S. . 7t�i-09, through the State of Minnesat�
under the Maternal and Child Act; and
WHEREAS, the Gity will best serve the needs of the target population
�nd �r�aintain program flexibility by providing services through a cost
reimbursement process in order to make services available to the gen�ral
community.
NQW, THEREFORE, in consideration for the mutual cavenants and promises
herein, it i� agreed:
I. DUTIES: The City shall p ovide thoae services which are described
in attachment I. , such service shall hereafter be referred to as
"Contractor 's Duties".
II. TERM OF CDNTRACT: This ontract shall be effective an the Ist day
of January 1988, and shall remain in effect until the 31st day of ^-��mber,
1988, ar until �anceled as provide herein.
III. CONSIDERATIDN� TERMS AN COIVQITIONS OF PAYMENT: /�c��

- � B. Reparting and monitoring. Th City will :
a. Maintain program and tatistical records detailing all services
provided under this Agreement. .

b. Develop and maintain record-keeping system to log informational
or referral requests. This sys em should be e�aintained on all applicants
for service.
c. Retain all records or a minimum of three years beyond the
termination of the Agreement.
C. The City agrees that it shall comply with all provisions of Minn. Stat.
Chapter 3b3 with respect to non discriminatian in employment and provision
of services under this Agreement.
D. The City shall at all times e an independent tontractor and neither it
nor �ny of its employees perfo ming services pursu�nt to this Agreement
�hall be deemed employees of the County 4or any purposes. The County shall
not be responsible for the payment of any taxes, either federal or state on
behalf of the City nor shall the County be responsihle for any fringe
benefits. Na Civil Service or o her rights of employment will be acquired
by virtue of City's 5ervi�es.
E. The City agrees to defend, hold harmles5 and p�y on behalf af the
Gounty, its afficials, agents a d employees any demands, �laims or suits
arising out of the City's premise or per4ormance of this Agreement.
F. DATA PRIVACY: All data co lected, created, received, maintained af
disseminated by the County for any �urpose in th� caurse of the City's
performance of this Agreement is overned by the Minnesota Governmental Data
Practices Act , Minn. Stat. Ch. 3 (1985) , or any ather applicable state
statute, any state rules adopted ta implement the act, as well as federal
regulations an data priva�y. Th City agrees to abide by these statutes,
rules and regulations.
G. AMENDMENTS: Any Amendments to this agreement shall be in writing.

Goal : To reduce ths inci ence of unwanted pregnancy among
- adolescents in � suburban Rams y Ctiu�ty. �
Target Population: High ris ., lvw income ti .e. less than 20Q% of
feder-al poverty g��idelines) dolescents in suburl�an Ramsey County.
t]bjective 1: To provide #amily planning clinics that are
accessible in terms of bot times and Iocation to law income,
high risk sub��rban Ram�ey ounty adolescents during 1988 and
19�9.
Methods:
1. Expand c!srrent clini�s to provide one zvening clinic per-
month at both Whi�e Rear Lak: ar�d Mounds View sites.
2. Exp�nd current clinics o pr'ovide one late afterno�n clinic
per week at both White Pear ake ar�d Maunds Vie� sites.
Objective 2: Tr pravide r pro�uctive health education to law
income, high ris�: suburb�n R msey Cc+ianty adolescents.
