The basis for the recommendation is as follows: On August 16 and August 31, 1999 you were sent letters from the Office of License, Inspections and Environmental Protection stating that your license application was incomplete. You were advised that you stili needed to provide a Tow Truck License A�davit, Tow Truck Vehicle Inspection SheeEs, a letter stating the location and security of the lot to which vehicles would be towed, a$10,000 surety bond with attached power-of-attorney, a certificate of insurance, a copy of the minimum non-contract fees, a list of drivers, with home addresses, a copy of the property lease or purchase agreement, and a site plan.
