scheduled for 3:30 p.m., Wednesday, November 24, 1999 in the City Council Chambers, Third
Floor, Saint Paul City Hall and Ramsey County Courthouse.
Enclosed are copies of the proposed resolution and other documents which will be presented to
the City Council for their consideration. This is an uncontested matter, in that the facts
conceming the failure to submit information necessary to process your license application and
the failure to submit evidence of liquor liability insurance or a waiver a�davit have not been

Dear Sir/Madam:
The Office of License, Inspections and Environmental Protection has recommended that
the above-referenced license application be denied The basis for the recommendation is as
foilows:
On September 13,1999 you were advised that your license
application was incomplete and that LIEP still needed to
receive evidence of liquor liability insurance or a signed waiver
affidavit, a copy of which was sent to you in August. As of
today's date, no waiver has been received nor does LIEP have
evidence of liquor liability insurance.
If you do not wish to dispute the matter, you must send me a letter admitting the facts stated
above and ask that the matter be set in front of the City Council to detemune what penalty, if
any, will be imposed. You will have an opportunity to appear and speak on your own behalf, or
to have someone appear there for you.
On the other hand, if you wish to dispute the above facts, you may request an administrative
hearing before an Administrative Law Judge. If you wish to have a hearing, you must let me
know that in writing. I wiil then schedule the hearing and send you a notice with the time, date
and place of the hearing, the name of the administrative law judge who will preside, and a brief
description of the procedures to be followed.

Please let me know in writing no later than Thursday, October 21,1999 how you would like
to proceed. If I have not heard from you by that date, I will assume that you are not
contesting the facts iadicated above. The matter will then be scheduled before the City
Council on what's called the Consent Agenda during which no public discussion is allowed.
Please be on notice that your business does not currently have a license to conduct a Malt On-
Sale operation at this location, and you are not permitted to operate a Malt On-Sale business
without such a license. You must, therefore, immediately cease all Malt On-Sale operations at
this location. If you continue evidence of that fact may be used in addition to other evidence to
deny your license.

Dear Sir/Madam:
The Office of License, Inspections and Environmental Protection has recommended that
the above-referenced license application be denied The basis for the recommendation is as
foilows:
On September 13,1999 you were advised that your license
application was incomplete and that LIEP still needed to
receive evidence of liquor liability insurance or a signed waiver
affidavit, a copy of which was sent to you in August. As of
today's date, no waiver has been received nor does LIEP have
evidence of liquor liability insurance.
If you do not wish to dispute the matter, you must send me a letter admitting the facts stated
above and ask that the matter be set in front of the City Council to detemune what penalty, if
any, will be imposed. You will have an opportunity to appear and speak on your own behalf, or
to have someone appear there for you.
On the other hand, if you wish to dispute the above facts, you may request an administrative
hearing before an Administrative Law Judge. If you wish to have a hearing, you must let me
know that in writing. I wiil then schedule the hearing and send you a notice with the time, date
and place of the hearing, the name of the administrative law judge who will preside, and a brief
description of the procedures to be followed.

Please let me know in writing no later than Thursday, October 21,1999 how you would like
to proceed. If I have not heard from you by that date, I will assume that you are not
contesting the facts iadicated above. The matter will then be scheduled before the City
Council on what's called the Consent Agenda during which no public discussion is allowed.
Please be on notice that your business does not currently have a license to conduct a Malt On-
Sale operation at this location, and you are not permitted to operate a Malt On-Sale business
without such a license. You must, therefore, immediately cease all Malt On-Sale operations at
this location. If you continue evidence of that fact may be used in addition to other evidence to
deny your license.
