WHEREAS, the Minnesota Department of He th desires to contract with eligible community
health boards for certain programs whic pr vide community health services to Saint Paul
residents; and
WHEREAS, the Saint Paul Board of Health 's he appropriate community health board for the
purposes of this contract; and
WHEREAS, the State of Minnesota Departm t f Health has requested that in disbursing monies
to the Saint Pau1 Board of Health that t e oard designate a fiscal agent to receive such
monies; and
WHEREAS, the Saint Paul Board of Health as requested that the City of Saint Paul serve as
the fiscal agent for the Saint Paul Boar o Health; now, therefore, be it
RESOLVED, that the Saint Paul Board of H al h supports the grant application of the
Saint Paul Division of Public Health and su sequent contract with the Minnesota Department
of Health for the Community Health Servi es subsidy and authorizes proper City officials
to contract on its behalf with the Minne ot Department of Health.

The State Health Department will fund a s s y of approximately $3.50 per Saint Paul
resident for select communicable disease n ol and health promotion activities in
Saint Paul. The Division of Public Healt w ts to apply for these State funds and is
completing the required plan update. Rec ip of these funds are already included in
the budget for public health.

- Up to $1,945,478. for the 1990-91 bienn um will be available for the provision of
select health services in Saint Paul. he Board of Health must approve the Division
of Public Health's application for thes f nds.
